A perfect on-page score is not a ranking
A page shows a green tick or 100 out of 100 in Yoast, RankMath or a similar plugin. It does not rank in the top 20 for its target query.

The score is a checklist of on-page items. It is not a prediction of position.
Before you check
Know the score measures your page, not your competition.
Have one target query for the page.
Check in an incognito window, or read Search Console rather than a logged-in search.
Diagnostic tree
Run the checks in order. Each check ends in a resolution or the next check.
Check 1. Is the page indexed?
How: Search Console, URL Inspection, enter the URL, read the coverage line.
If the page is not indexed, go to Resolution A. If it is indexed, go to Check 2.
Check 2. Does the page have any impressions for the target query?
How: Search Console, Performance, Search results, Queries tab, search the target query.
If the page has zero impressions for the query, go to Resolution B. If it has impressions, go to Check 3.
Check 3. Where does it rank, and who is above it?
How: read its average position for the query. Then look at the pages ranking above it.
Why: position is decided against the pages competing for that query, not against a checklist. The checklist cannot see the competition.
If it sits at position 11 or worse, go to Check 4. If it sits at position 1 to 10, go to Resolution C.
Check 4. Is another of your pages competing, or is a competitor covering the intent better?
How: check whether a second page of yours targets the same query. Then assess the top three competitors on intent fit, depth and authority.
If another of your pages competes, go to Resolution D. If a competitor does, go to Resolution E.
Resolutions

E. Outranked | A competitor covers the intent better | Improve intent fit, depth and internal links, then re-check in 30 days |
When to escalate
Escalate when a page has no impressions for its target query 30 days after it was published, and the query has confirmed demand.
What the score does measure
The score checks your title length, your meta description, your heading structure, your keyword placement and your link counts. Those are real signals, and a failing score can hold a page back.
What the score cannot see is everything that decides the ranking. It cannot see your authority against the competing pages. It cannot see whether the page matches the intent behind the query.
It cannot see your internal link depth. A page can pass every check and still lose to a competitor who covers the intent better.
